ACORD Changes Claim Report Forms

​Insurance industry standards-setting organization ACORD has announced new editions of three forms commonly used to report claims.  Big I New York members should plan now to implement them.

The revised forms are:

  • ACORD 3 (2019/09), Liability Notice of Occurrence/Claim
  • ACORD 4 (2019/09), Workers Compensation - First report of injury or illness
  • ACORD 831 (2019/09), Professional / Specialty Insurance Notice of Incident / Claim

All three carried prior edition dates of 2016/10.

In addtion to cosmetic changes, such as ACORD's new logo, ACORD 3 and ACORD 831 have replaced references to "Agency" with "Producer." All three forms have also revised statements applicable only in Colorado and Puerto Rico, and deleted a statement that was applicable in Hawaii. On ACORD 4, a statement that is applicable in New York has been moved above the employee signature line on the third page.

All agents and brokers should begin using the new editions on September 1, 2019.
